Information about the services provided by Madison County Service Coordination

Populations Served

Madison County residents with a developmental disability living in the community or receiving DMH funded residential services from identified providers are eligible for TCM services.


Typically, individual homes, agency offices, or other community location of the individual’s preference.

Days & Hours of Services

Services are provided based on preferences and availability of individuals and their family. Office hours are M-F, 8:30 am – 4:30 pm. Support Coordinators are available outside of office hours through the after-hours response system or prescheduled meetings with individuals and families.

Frequency of Services

The frequency varies based on needs identified in the Individual Plan and individual preferences. Considerations include service monitoring requirements established as part of the Medicaid Home & Community based services the individual receives.

Referral Sources

Anyone can complete a referral. Our top referral sources include:

  • public schools- including teachers, counselors, First Steps (early intervention) and early childhood programs
  • family members
  • therapists
  • physicians
  • state agencies, such as Children’s Division, Health & Senior Services, Vocational Rehabilitation

Description of Services

Each individual served is assigned a Support Coordinator. The role of the Support Coordinator includes:

Planning: Support Coordinators facilitate the development of an Individual Plan to address the individual’s and family’s unique support needs and preferences.

Linking: Support Coordinators connect the individual and/or family to a variety of resources, including community resources, purchased services funded by the agency and/or services available through a Home & Community-Based Waiver.

Monitoring: Support Coordinators monitor and assess the quality, timeliness, and effectiveness of services and supports.
